What causes severe pain weeks when medicines are inserted through an IV?

I had an IV inserted at the ER and every time the nurse put medicine in it, the pain was horrible.  I told the nurse and he looked at it and said it was inserted correctly. Now 3 weeks later I am still in horrible pain, from my shoulder to my hand, especially around my elbow and forearm.  I even thought I was having a heart attack and went back to the ER, they said I had a pulled muscle and sent me home, that was 2 weeks ago. Yesterday I started having tingling in my hand. They gave me muscle relaxers and I already had pain pills, neither one helps the pain, what should I do?
