What is this pain? Can it be nerve related?

I'm male, 37 years old.   Couple of years back, I noticed mild to moderate pain in my left foot.   When I went to my GP, he told it is just an inflammation due to cold weather and asked me to take ibuprofen.

However, for the last 8 months I'm suffering from moderate left testicular pain which started as dull pain on my left testicle. I'm also getting pains in my left knee and left foot.  I have pain in my left palm as well with occasional numbness.  When I sit, it is hurting more than how my left testicle was hurting in the past.

Sometimes I get severe headache on any one side, and if I touch on top of my head I can see pulsing with pain.

Since I had more pain in my testicle, I was seeing my urologist.  All the STD tests came back negative.   I don't see any visible changes in my genitals and no lumps or lesions.  No discharges and Urination frequency is normal as well.

During a recent color doppler ultra sound scrotal examination report came with the following impression "Mild Left Epididymitis? with Bilateral Mild Hydrocoele and Left Inguinal Adenopathy".    I have no idea why the report was having a question mark after epdidymitis, may be doubtful?

No evidence of varicocoele.
Bilateral minimal hydrocoele.
Left inguinal discrete adenopathy noted.
Testis: Normal in shape, size and echotexture.   No evidence of focal lesion. Normal intra-testicular vascularity demonstrated."

After seeing the reports, my urologist was saying I have inflammation due to some infection, and started treating me Doxycycline.  However after completing the 10 days, I still have the pain.

I feel the pain actual pain originating on top of my testicle (backside) and the base of my penis (backside), the place where penis and testicle joins.   I'm able to roll my left testicle which doesn't cause any pain.   I'm thinking can this me more related to some injury or infection in the nerves in this region, rather than epididymitis?

May be I injured it this region due to excessive cycling/workout?  Can injuries also cause inguinal adenopathy and hydrocoele?   I can feel the pain like pulsing pain in my left palm and left side of genitals.   Sometimes I see my testicle pain shifts to right side, at this time there is no pain on my left side. Can this be a nerve related issue?

I'm suffering with this pain for 8 months now, and it has become chronic.   Can somebody help me understand what exactly I'm undergoing, and how can it be treated?   Please help.

I have several blood tests and various cultures taken for several times in different labs.  All the STD reports are negative, and the observed readings are in fact too small.

I'm also having pain while pressing on my left head as well as I'm getting severe headache on one side (mostly of the time on left side).  I get pain even behind my left eyes.   Sometimes, the pain and numbness in my left hand radiates upto my left cheek.

MEDICAL PROFESSIONAL
Hi there. epididymitis is a medical condition where there is inflammation of the epididymis. This may be mildly to very painful and scrotum may become red, warm, and swollen. This is the most frequent cause of sudden scrotal pain. Doppler ultrasound scan has been done for you and the radiologist reported query epididymitis. Which means it cannot be confirmed on scan if it is so. Microscopy and culture of the first voided urine sample and gram stain of the fluid from urethra to check for any sexually transmitted diseases.  A deep vein thrombosis on the left side and demyelinating lesions like multiple sclerosis responsible for the multiple neurological symptoms needs to be ruled out. Take care.

Also,  nearly 3 years back,  when I got up from somewhere, my left head hit against something very hard.   The pain was terrible and no swelling.  Since I din't have any concussions,  I din't goto a doctor, and the pain subsided in a day or two.    However after few months the pain started on my left foot.    I'm thinking whether are all are interconnected.

I have more pain and numbness in my left hand.

Now my urologist is treating me thinking it is epididymitis.    But I still have the pain for several months now.    The pain is constant.

Can this be neuro related rather than checking with urologist?
