It depends.

If you possess a CDL and are operating under DOT regulations the answer is no. You may not drive, even with a physician's note.

If you are operating a civilian privately owned vehicle, the issue of impairment would be brought up. If you were in a fatal accident and underwent mandatory blood testing, you would be in a very unsatisfactory situation. A low dose of this medication in someone who has used it for a while would not necessarily cause impairment. Nevertheless, some states have a zero tolerance for opiates. You should discuss this with your physician and if you must drive, request he write a note.

gymdandee's #3 is good advice.

A message from the pharmaceutical:

"Use caution when driving, operating machinery, or performing other hazardous activities. Suboxone may cause drowsiness, dizziness, or impaired thinking. If you experience drowsiness, dizziness, or impaired thinking, avoid these activities."

Now, legally, you can drive or operate machinery while taking Suboxone.  However, should you ever be in a serious accident, the police officer and/or prosecutor could easily make claims that you were driving under the influence, weren't using caution, and should'nt have been driving in the first place.  You could be in deep doo-doo.
