orthopedics-pott's spine

Hi doc,
            My father ,aged 50 yrs is suffrng frm pott's spine since past 4 months.....4 months ago he had severe back pain for which he tried all kind of physiotherapys ,as misdiagnosed...was under steroids for a month...nd when d pain did not subside we went for other doc...where he diagnosed it as Pott's...
TB Gold test was positive, ESR was 90..MRI showd infective spondylodiscitis with L2-L3 region with intervrtbral disc almost necrosed..
Now aftr 3 mnths of AKT4 drug therapy he feels slightly relieved as far as pain is concernd...ESR has fallen to 44...but MRI says infective pathology has further progressed..
I cannot relate the clinical symptoms with the MRI ones...
I would be grateful if u could help me....

M thankful to u dr.kaul for giving it a concern...

the MRI reports mentioned that "infective pathology has progressed" so it clearly suggests that it is an infection ...on d other hand which other pathogen can cause this damage...

is sample aspiration mandatory from the site at this stage???????
n his pain has subsided to 5o percent of that it was earlier ....cudnt further degeneration would have caused more pain???

he is not on analgesics and anti-inflammatory drugs ...

kindly help..

Hi there!

It would be difficult to comment on the situation without having had a look at the MRI images. The pathology described on the MRI may not be due to the infection per say but could also represent further degeneration which could occur even after the infection has been controlled. It is also possible that the medications have not been able to control the infection or the lesion is due to a different infective source. It would be best to discuss the MRI results in detail with his treating orthopedician and if a change would be require in his current treatment plan.
