6cm endometrial cyst

I'm 37yrs old i have 6cm Endometrial cyst  the cyst was found by TVS . my question is it really need an operation? my ob gyne advice me to undergo an operation,  there is no other test before i undergo an operation like biopsy first before operation? how accurate the transviginal ultrasound?what other action i should do before i undergo an operation? i need an advice pls.thank you...

I am 15 yrs old and i have 6cm Endometrial cyst. It was found by pelvic ultrasound. I was referred to have an ultrasound because I have dysmenorrhea. any advice? thank you

Is your cyst complex?   Any cyst that is 5cm or larger and complex should be surgically removed to rule out malignancy.   Even benign cysts tend to cause problems the larger they grow over 5cm.  Usually, a gyn will order a MRI or CT to get a better picture of the contents of the cyst and to see the entire pelvis before surgery.  

If your cyst is complex, then you should also ask for a consult with a gynecologic oncologist.  This does NOT mean it is likely it is cancer.  It is just a precaution because gyn/oncs have special training for removing malignant cysts.  They are less likely to spread the contents during surgery (which keeps you at an earlier stage of cancer), and they can completely stage you in the same surgery (removing lymph nodes, omentum and other tumor spread if needed).   This helps you get more accurate and timely treatment in case a malignancy is found.

One final test you should request is the CA-125.  Although it is not a good tool for diagnosing ovca (gives both false positives and false negatives), it is great for follow-up after you have been diagnosed.  Watching your CA-125 numbers can help assess how well treatment is working and as numbers go up it can indicate recurrence.  Having a baseline pre-surgery number will let the doctors know if this test will work for you in the future.  Also, you can see how well your numbers go down after surgery, chemo, etc.

Ok...that is all the "cancer" information.  But, do remember it is very unlikely that you have a malignancy.  Cancer experts agree this is the safest route so you are not left with an inadequate surgery.  However, almost all large complex cysts still turn out to be benign.

I was 37 with a 6cm mass when I was diagnosed with Stage 1a ovca three years ago.  I am so thankful that I learned about the importance of a gyn/onc right here on Medhelp.  My gyn referred me to one without my asking, but I made sure that I called my insurance to help the process along  once I realized the very important nature of having one.  Here are a couple of links, FYI:

You can also google about gyn/oncs and find a lot of valuable information.  Please try not to worry despite this long, drawn out recommendation list.  Some gyns try to do the surgery on their own and will only refer you if you ask.  It is better to know all the info. so you can make an informed choice.  I hope all turns out benign and keep us posted on how you are doing!
