Fluid level, movement, growth (sometimes they worry that your baby will be too big to deliver vaginally). If they say baby is too big, I'd ask for a trial of labor anyway. Back when it was "once a c section, always a c section" my aunt was forced to have a preterm section because her baby was "way too big" to deliver vaginally. She ended up being barely five pounds. Because my aunt had one surgery, she had to be sectioned for all three, all of which were tiny!
My sis in law it's just as petite as my aunt and she's had three vaginal births, two of which were eight and a half pounds!
And what does the ultrasound look for?
My non stress test was basically getting hooked up to the same monitors the hospital has. There's a Monitor to watch frequency, duration, and intensity of contractions, and one to watch baby's heart. My baby wasn't stressed but I'm sure they'd probably induce immediately if baby is showing signs of distress, especially since your due date has passed.
