Suggestions for birth plan?

I'm going to start making my birth plan. I know there are some on websites that have printable birth plans, and I was wondering if anyone has a suggestion of which websites have the best ones to print and fill out?

*theirs was. God my head is all over the place today! It's good for a base and has lots of ideas, things I hadn;t even thought to write but would want in mine anyway.

I don't know any websites, not looked into yet but there is this group on here about natural births and this was one of the discussions and they shared what there's was. It looks simple and easy to read to, just heading and bullet points so would be easy enough to type up if you couldn't find one you liked on a website. Here's the link if you fancy a look.

Sometimes your ob doctors office will have a birthing plan that you can fill out.  I dont know about websites though.
