Premature Ejaculation except on my back

Hi,I'm a 28yr old caucasian male.I'm about 57' 195,I'm a smoker not in great shape,but not awful.Me and my wife have been together for about 4 years.
For about the first 2 yrs we were together our sex life was quite good.Then two things happened we quit using condoms and I had an issue with hemroids.I really don't remember what came first I know they happened around the same time.

Now when we have sex,I finish very quickly unless I'm on the bottom on my back.When I'm on the bottom I usually last anywhere between 15mins to 30mins,ocassionally stopping breifly if I need to.When we try any other position I finish in no more than a couple of minutes usually less.This did not happen years ago when we were using condoms and never happened in past relationships.We have tried numbing agents and it doesn't seem to help much at all and it also numbs her.I believe if this was a mental problem or performance issue it would happen all the time,but when I'm on bottom no problem.

I also had a problem with hemroids a couple of years ago.I had signifigant discomfort and quite of bit of bleeding.I had a colonoscopy and was told I had internal hemmroids,I changed my diet and havent had any signifigant problems since.However,ever since then whenever I have a bowel movement,when I wipe there is just a little bit of fresh blood on the paper,no pain or discomfort really.And I have never had anything but a enema up my anus,so please don't make that suggestion.Not meant to be disrespectful of others,I just have not so I know that isn't the issue.

I have also noticed over the last 2 years or so that I have some trouble urinating,no pain,just a infrequent weak flow.Often times its hard to start urinating,the stream is weak and starts and stops often,also when I finish I tend to dribble more than normal.

Does anyone know anything that could help,I can see a doctor in about a month as my insurance from my new job will kick in.But can anyone help in the meantime,this is getting very frustrating for me and my wife as well.

Does this sound like a prostate issue,a mental or performance issue.Should I try some male performance enhancers,please I'm open to suggestions.                             Thank You

No we have not tried going back to condoms,but I will make the suggestion and give it a try.I know that not using condoms means that there is more sensitivity and therefor excitement which could mean earlier ejaculation.However like I said when I'm on my back on bottom I have no issue,I can control how long I last quite well.

As far as the hemmroids they have not really been an issue since the outset of them years ago,but there has been a little blood 99% time I wipe since then.

I just thought with the sex problem,the hemroid issue,and having a weak unsteady urination stream that is often kind of difficult to start in the first place,added up to a prostate problem.I sincerely hope it is not as I'm only 28.

I thank you for your advice and will try to go back to condoms as test.I desperately want to fix this issue so that our sex life does not become a major problem in our marrige,as I love my wife very much and want us to be happy.If you think of anything else please let me know.

It sounds like a possible prostate problem. I'm glad to hear you will be seeing a Dr soon.
The other thing that may be going on is not using condoms any more. You were used to that and now there is probably alot more sensitivity. Have you tried using them again to see if it helps with different positions?
As far as the hemorrhoids, there are ointments and creams to help shrink them. Ask the Dr which one you should use when you see him.
