What should be my line of treatment and what is the diagnosis?

My path levels are:
TSH - 4.88
ESR - 40
Total Cholestrol - 266
HDL - 61
LDL - 168
VLDL - 37
LP (a) - 58.42

I will be very grateful to receive some guidance as I have been really feeling very unwell. My weight is 110 kgs and Iit semmed that I was continuously gaining more weight. My body had become sluggish, my mind foggy, had heart palpitations, no sleep, no appetite, extermely tired through the day, bouts of depressions and feeling low, swollen ankles, eyes drooping and so much more......
I am worried. So seeking help. Thank you.

The thyronorm may begin helping your symptoms, though it takes 4-6 weeks for it to reach full potential in your blood.  You still need the above mentioned tests, and if you have Hashimoto's, you should plan to be tested often and will probably have to adjust your medication periodically to accommodate declining thyroid function.

Hey! Thanks much. Will get the results and post again. Meanwhile visited doc, started me on Thyronorm (25 mg) as I am hypothyroid as you already said...

Your TSH is somewhat high, indicating that you might be hypothyroid, but you need to have Free T3 and Free T4 tested to confirm.  You should also have thyroid antibodies tested to see if you have Hashimoto's Thyroiditis.

"My weight is 110 kgs and Iit semmed that I was continuously gaining more weight" ----- are you still gaining weight?  All of your symptoms, along with the high cholesterol level indicates hypo, but again, you need further testing to confirm.  Talk to your doctor about having the test suggested above, then post the results and reference ranges, here.
