do I need paratyroid surgery

I am 58 years old and have had elevated calcium levels on and off for 10 years since I was treated sucessfully for Grave's disease.
Currently, my calcium is 10.9, PTH 111, bone density scan reveals osteopenia. My vitamin D level and results of 24 hour urince culture were normal.
What is next step for me?

A specialized bone endo is the one that really treats this condition. If you continue to have issues, I would contact one of them - but there are only a few - I only know of them in NY and CA. They do run clinical trials so it may happen that you may qualify.

Yes, I have come to that conclusion as well. I have a follow up appointment with endo doc in a few weeks. We will have that discussion and I will seek new MD if he continues to "watch and wait".

I would find a competent doctor and get treated. The tumors can be tiny but it sounds like it is effecting you and your bones!
