Everybody with toes has metatarsal joints. It's just an anatomical term. Nobody on the radio or on the internet can do what is necessary to help you, and that is to examine your feet. I believe a podiatrist is the proper doctor to help you, and if not a podiatrist, then an orthopedist and a physical therapist. It will probably take antiinflammatory medications plus certain exercises and proper diet with very little sugar and lots of vegetables and lean protein to get you healthy, but it can probably be achieved if you can commit to a program. It won't be a magic pill or even surgery, probably.
Sorry, you've probably had countless xrays. Just wondering if there is any chance of an old, untreated metatarsal break that is acting up now?
