For such dull pain radiating to the abdomen, any hernia and infections of the testicles or scrotum should be ruled out. It can be due to epididymitis. This is more common in young men and may cause a heavy sensation in the testicle area. The area becomes more and more sensitive to pressure.
You need a physical examination from an urologist along with urine analysis, complete blood count, testicular ultrasound and scan and tests for chlamydia and gonorrhea. Treatment will depend on the cause.

There can be several reasons. If it happened suddenly I expect it to be an infection of the testicle, orchitis, or of the epididymis, epididymitis.
I used to end up with it about every three to four months. It starts with a dull ache but can get worse very soon. In all cases waiting is not wise because these infections can do serious damage to the reproductive system.
You did not mention your husband's age, but one allways must rule out testiclular cancer. The fact that there are no bumps or lumps makes the possibility of cancer a lot less.
Does touching or lifting the testicle cause pain to increase?
In my situation I allways ended up being hospitalized for four days to a week and was given neumerous antibiotics by IV.
In 1999 I came accross an article which mentioned that males undergoing a prostatectomy used to get a vasectomy as well to prevent infections from spreading to the testicles. In my early 20's did I start having problems with kidney infections and prostatitis. So in 1999, after my wife and I had four children I had a vasectomy. It did not go smoothly as I ended up with a hematoma on the left testicle. 2 days after the vasectomy I had emergency surgery to have the blood clot removed. It did not heal well and a year later I lost my left testicle due to a Strep B infection.
The good news is that I have had no problems with the right testicle sinse.
