pain while urinating

Hi I am nineteen yrs old and for the past six months I've had pain while urinating and just a month ago foamy urine development. The urinalysis was normal. A renal ultrasound performed was also normal as was the results of a prostate exam. The urologist said not to worry about it. I also have hyperacidity and black stools from time to time as well as a constant (more or less) pain on my upper right stomach. Probably an ulcer. I'm taking pantoprazole for it now. The urinary pain is always there just a bit above the base of my penis. The urologist said it sounds like a bladder problem.

Sometimes I get rashes, most likely because of a mold allergy.

If you need more information please ask. What advice can you offer me?

Yes I agree there must be a cause of this painful urination. When I pee in a cup the foam is not there as much, only in the water. The GP said it doesn't mean anything and since the urinalysis detected no protein in the urine I doubt it is of much concern except that it started recently and although it seems harmless I suspect it is directly related to my painful urination.
I was worried taking pantoprazole would harm my body. I've been having hyperacidity for the last year. I took it daily when I had a constant and very painful ache on my right side right below my chest. The ache went away after two weeks of pantoprazole use after which I discontinued it. Since then the ache has returned from time to time as has the hyperacidity. I was taking pantoprazole but not regularly. The black stools seemed to be correlated with the amount of hyperacidity I experienced. Now I am taking it daily. Just this morning my stool was brown with black "dots."
I have thought long and hard about the cause of the painful urination. I used to take clonazepam recreationally several years ago in high doses once a week, approximately 3-5 mg. I smoke marijuana occasionally and that always makes it much much worse. I use methylphenidate rarely when I have exams. I am a two cup a day coffee drinker. Coffee seems to make the pain worse as does a full bladder. The pain is most intense right before I urinate. Cigarretes also seem to make it worse.
I used to take valerian root, a herbal extract in capsule form for a month or so right before the hyperacidity started. During that time for a week I went on something called the warrior diet where I would eat nothing during the day until six pm and then I would binge on a feast. This was not for weight loss. I weigh 180 pounds approximately and am 5'9. I think the ache in my stomach started when I was doing that and I stopped the diet.
I take fish oil capsules every day.

A month and a half ago I took gabapentin to try to relieve the pain. This had the opposite effect. Is it significant? Since gabapentin is excreted by the kidneys could my kidneys be damaged in some way due to my irresponsible actions in the past?

A blood test showed creatine and BUN in the reference range. The GFR was normal. However I calculated the ratio of BUN to creatine and found it to be about 44. Does this mean anything?

I am wondering if the hyperacidity could be related to the painful urination? Although all the tests done so far seem to be normal perhaps more tests can be recommended? I did the h pylori test and that was normal, no bacteria.

I am very frustrated with my body. Please help me as best as you can. Are there natural remedies (ex. hot showers, exercise, meditation, etc.) that can help? What clinical approaches might benefit me?

Hello,
I agree with the urologist that if the renal ultrasound is fine, then bladder causes like cystitis etc needs to be thought of. this can be confirmed by cystoscopy of the bladder. The black stools are due to blood in stools. The causes of bloody stool that is more maroon in color is intestinal polyps, peptic ulcers, inflammatory bowel disease or diverticulosis (diverticulosis causes significant bleeding). I suggest you to consult a gastronetrologist and get these evaluated.

Hi,

My first question is, what were you prescribed the Pantoprazole for. I use it myself due to a hiatus hernia. Acid indigenstion can cause pain but that is normally higher up. It also causes irritation of the esophagus.

Painfull urination can be hard to explain but there has to be a reason for it. Do you mean that when you unrinate there are air bubbles in your urine. I mean is it different from the urine causing foam when you urinate in the toilet from a distance?  If you were to urninate in a container is the foam different?

I recently ended up with black stools and found out that there was blood in the stool. Did you have a stool sample analized? In my case they thought that it was due to an infection.

Did the urologist have you give a urine sample? If you have a UTI, urination can be painfull.

I don't have any answers for you but I have simmilar symptoms. I have had numerous UTI and kidney infections and have experienced pain from that.

It sounds to me that the urologist is not doing his job. If he thinks that it is a bladder problem, he should infestigate things further and not just dismiss you as just a bladder problem.

I experience pain sensations in the area between the penis and belly button. Having been diagnosed with a neurogenic bladder, and having had years with abdominal cramping, it is suggested that this is due to a neurological disease that I have.

I think that you should get a second opinion.
