My period has been going on for 12 days and won't stop.

I've been having my period for 12 days straight now, it is extremely heavy with lots of blood clots. I have been wearing the overnight pads that are supposed to be able to last up to 10 hours and going through them in an hours time. I had a baby 9 months ago, and my periods have been every other month but never anything like this. What could be wrong?
