Hi there and welcome to the forum.
I agree that it is time for a full pelvic exam and pap smear with an ob/gyn. I am one to ask friends which ob/gyn they go to as well as I look for offices that are close to my home--- then I double check if they are covered by my insurance. Are you still on your parent's insurance or do you have your own or how do you get your medical care? That can be a deciding factor in who you end up being seen by if you don't currently have an ob/gyn. If you find yourself without insurance, Planned Parenthood is a good resource for young ladies needing gynecological care--- check for the phone number and address in your city. (often in the downtown area of cities).
But you need to be seen.
I do want to say this. I'm sure you know this but the pull out method is not a birth control method. If you are having vaginal intercourse and the man pulls out, you are risking pregnancy. Please buy some condoms or discuss birth control with your care provider when you have your exam. Totally worth it so there are no accidental pregnancies or undue anxiety. You can't always rely on the man for these things so take care of this on your own.
As to spotting for a long time, this can happen for a number of reasons including a hormonal imbalance such as too much progesterone. But your doctor will have to work on the reason why to give you any definite answer.
good luck dear and let us know what the doctor says.
If you are seeing your PCP for these issues, I strongly suggest you find a good OB/GYN and get a second opinion, preferably an OB/GYN specialist.
Your local hospital should be able to refer you to the nearest one.
I can understand your frustration and confusion as it definitely appears something is going on.
You may need a CT scan to look for a structural abnormality which could be causing these problems and which wouldn't show on standard blood tests or PAPS.
