Ketotic Hypoglycemia

My son has been diagnosed with ketotic hypoglycemia.  He too is very skinny.  He also complains of palpitations and his heart rate goes to about 160-180 by holter monitor but the cardiologist said his echo was normal and the monitor although it showed a fast hr it was sinus rhythm.  When he stands up his heart rate goes faster but he is not dehydrated, we keep him well hydrated and his labs don't show dehydration.  (nor a fever).  In my family we have many autoimmune diseases.  I have Sjogren's and Raynauds.  My Aunt has Autoimmune hepatitis, primary biliary cirrhosis, Sogren's, Raynauds, and Rheumatoid arthritis; my cousin has Hashimotos and Sjogrens.  My question is this, is it possible my son has autoimmune hypoglycemia?  His insulin level was 2.8 when he was fasting.  He also has low muscle tone and gets PT/OT, reflux, constant upper respiratory infections including 5 pneumonias and was hospitalized twice for pneumonia.  When his glucose is low it can go to 28.  It is especially more profound when he is ill - which is about once a month.

I also forgot to ask, does your child suffer from migraines because of this? My daughter has them now daily.

I would love to see an answer to this. My DD is hypoglycemic at 6 with unknown origins. We will be going to CHOP for further testing to find the cause. She can drop from 137 (through food we can get it high) to 37 in less than 30 minutes, we have been told she rapid drops. She spills ketones and we have Hashimotos, Addisons, Type II Diabetes and Pituitary tumors in our family and all came back normal (clean MRI of brain and pituitary). This roller coaster is scary for all involved and it seems the information is so limited for this age group. The lowest my daughter has gone is low 30s and each time it seems to go lower faster.
