bipolar and dull

we have bipolar patient ( young male ).diagnosed for bipolar (type 1) for 10 years. hospitalized when his disorder start since  he's  talking pills for 10years.but few months ago his manic episode reappeared  and  he quiet his job  now he's  on lithium  and clonazepam for 3 months and his manic episode under control
but sometimes he's acting like he know everything , dull,dramatic.lying, and talking unrealistic event,tell his  secret things to others
sometimes he' look like a normal person
do you think taking  drugs automatically fixed his unrealistic thinkings or he need counseling  etc

Medications help but do not cure or prevent episodes with 100% accuracy.  He needs counseling to help him recognize triggers and early symptoms so he can avoid episodes or get help before they get as bad as the current episode he is in.  He may need a medication adjustment as well.

I do not feel that the drugs ever fix anything.  Medication is an aid or a tool that makes it a little easier to work on things but if we do not do the work the only thing that changes is that we hold of problems that were not resolved to begin with.
I do think that counseling or therapy is essential for most people when mania breaks though again.  We can go for years without an episode but once it happens just a change in medicine is not always effective.
The real question is why did his episodes return?
A change in where he lived, a change in his relationship, did he start using substances other than his medicine, or stop his medicine?
There as a reason for the episodes to return after ten years usually change like loss or gain.
