intellectually disabled son

my intellectually disabled 13 yr old son pees and poops in his underwear on purpose then puts them under his pillow to sleep.i am very concerned about the hygenic aspect of it as well as are we the only parents dealing with this? it has become almost daily and is deeply concerning us.please help us
