Who Do I Turn To?

I am currently on my second try at Lupron.  I was first on it for adenomyosis.  My gyn then performed a supracervical laproscopic hysterectomy with great relief for a while.  Then I developed more abdominal pain.  GI said epiploic appendagitis until a second ER CAT scan showed a mass with reactive lymph nodes.  GI sent me to a heme/onc surgeon who successfully removed the mass via lap,  Thank god mass was not cancer, but confusingly, it was an endomtrioma!  If endometriosis is defined as uterine tissue and I no longer have a uterus, how is it possible?  Had I known I would have had my ovaries out at the same time as my second surgery.  My new GYN (they are scarce now in these parts) wants to take out my ovaries to relieve the bowel pain and symptoms now present on my left side.  I cannot tolerate a third surgery at this time.  Who can afford to take off so much work?  This is getting too long, but the Lupron helped for a little over six months but now I suffer from cyclical left upper quadrant pain, severe bloating/gas, anorexia and nausea.  My question is who do I schedule surgery with?  Do I request that a bowel surgeon be present when I have my ovaries removed? Why can I find no specialist in my area?  I live in PA and would really like to find a skilled surgeon so that I can resolve all of this.
