why am I only on Sovaldi & Riba?

After reading a post this morn, I wondered why my doc only has me on Sovaldi & Riba ?  I just finished 12 weeks and am starting  next 12.  Yes 24 weeks and thankful no interferon, I am geno1A not to much damage at least no cirrhosis. Side effects rash, some hair loss, and a little insomnia. I still don't know why no OSOLYIO  any  guesses ?

i am type 1a..had vl of about 1.2mil
starting week 4 of S/O.
UND at 2 week lab!
we chose this because i have hiv (UND and good labs with that!)

  I have decreased kidney function due to doctors in past made poor choices of hiv meds, did not adjust dosage for my creatinine clearance (they missed it and never told me i had a low eGFR) ugh!

liver is good. grade 2 fibrosis alt and ast. good..

probably got hcv in 1980's

i have been suffering from chronic muscle joint pain and horrible fatigue that started about 3 years ago..  dumb doc took me off hiv meds and when i re started a reaction to hiv meds caused this pain syndrome.. been getting better but slow…today a lot of pain happens when i exert myself..like cleaning ,cooking,  and chores .can't work.

also going through menopause!

we didn't need to rock the boat with riba or int.
made sense ..insurance went through. saved me all the side effects that i do not think i could handle at all..

There are a lot of individual treatments based on various factors with Hep C. It starts with genotype, prior treatment experience, cirrhotic or not, and other associated risks.  A hepatologist would be your best choice for determining treatment options.  I am genotype 2, and treated with Ribavirin and Sovaldi for 12 weeks.  My particular type of virus does not respond to Olysio so none were given.  I'm not familiar with treatment plans for 1A, but others on this site are and will give you some insight.   Olysio is not always recommend for all.     Take care.    Kim

Thank You for info. I have never treated before but doctor felt if I chose to wait that he would not push me.  I felt good about treating at this point because I just didn't think I could treat with interferon, I'm in a position that really takes a lot of brain work and concentration and well I need my insurance.  I was undetected at my first blood draw which was 28 days into my therapy, They have stayed that way since.  My other numbers seem to be good, but there was some confusion with labs so  my next draw is on the 26th and I will be able to post numbers then.. I just felt that he should have tried with the OLYSIO.  My insurance would have approved I'm sure.   I wish everyone Here a healthy Recovery and a quick return to NORMALCY . Hi Kelly,

Someone may be along shortly to post some links regarding your treatment protocol.  In the meantime congrats on having completed half of your treatment.  How are your PCR's?  When did you become undetected?  

I too am GT1a but cirrhotic.  Also treating for 24 weeks but with S/O + Riba.

Have you treated in the past with Boceprevir or Telaprevir?  Olysio is also a protease inhibitor but with different properties.  Prior use or failure of previous PI's shouldn't preclude using Olysio.  

Did you screen for Q80k polymorphism?   Roughly up to 48% of HCV 1a's have polymorphism that some studies show may reduce the effectiveness of Olysio.  

The FDA just approved both Sovaldi and Olysio last fall for use this year.  However, their use together is considered off-label.  Is it possible your insurance company denied the request for the 3 drugs?  That would be my first guess.

The negotiated fee from my insurance company to the specialty pharmacy for Sovaldi and Olysio is $50,000.00 per month!  For 28 pills of Olysio alone is $22,320.13, Sovaldi is the most expensive at $1000.00 per pill.

There are a number of members on here that are on your S/R course and hopefully they'll chime in.  You are taking Sovaldi, that is the key to the success stories we're starting to see.  

Thankfully you are able to treat now with the exceptional drugs you are taking without having progressed to cirrhosis.  Your odds for curing HCV forever is much more likely.

      have you treated your virus before?  

   sovaldi/riba is a retreatment option for geno 1a or b. can you take interferon?

    sov/olysio is not an approved tx.  insurance may approve for patients who are geno 1 interferon ineligible, with advanced liver damage. also patients who are in unique liver populations(read ...hcvguidelines/full-report-view
