Welcome to the MS forum. I'm sorry you are in pain. While I can't weigh in on what your MRIs mean in reference to your pain, I can speak to something we talk about a lot here.
And that is, to urge you to please, move on to another Dr. if the Dr. you are seeing does not care, or you feel you are not in good care. Besides what the reports suggest, has the Dr. said anything to you regarding these results and what you have going on clinically? Please do inquire if you have not done so already.
You will learn a lot about MS here, and we are a very supportive bunch. Some are familiar w/Chiari, and if you would like to read through past discussions, just got the "search this community" box and type in "chiari" or "small vessel" etc.

Hi, you are more than welcome to stick around here and learn as much as you can, and I hate to refer anyone off to somewhere else. But I just checked the neurology forum here on MedHelp and did a quick search of chifari and there is a lot of information there specific to your disease.

5 mm for a Chiari is considered clinically relevant and your neurologist should be paying attention to that. It is operable if it is causing symptoms.

Chiari Malformation is a real deal. There is a forum here on MedHelp specific to Chiari Malformation, symptoms, treatments etc. I think you should be reading up on this and consider whether a new neuro should be in your future.
You have a single lesion in the brain and that should be looked at in rleation to symptoms you are having.
Proptosis is a bulging of the eye(s) from the socket. This can be a normal variation, but it also can be due to hyperthryoidism - think Barbara Bush. Your doc should run a thyroid panel on you. Other causes, like a tumore or abcess behind the eye would have been seen on the MRI.
Neither the Chiari nor the proptosis would be relevant to MS. The single lesion could conceivably be. But, the first two should be checked out.
