Help Please

What would cause my thyroid tests to come out showing hyper and hypo results? I have multiple nodules on my thyroid at one check and none at another ( I have been biopsied, nuclear scanned, ultrasounded, Cat scanned and blood tested repeatedly) Iam currently seeing a wonderful internest, that reerred me to an endo to set me up for Radioactive iodine abalation. My doctor felt there was no amount of medication that would "fix" my issues....about 2 yrs ago, I was 320 pounds and in 9 months I dropped 140 lbs. The only thing "they" could come up with was my thyroid...my previous physician put me on cytomel, refused to send me to an endo and would increas my dose if I lost any further weight....it "kind of" worked and my weight loss stopped, I had three nodules on the lower leeft lobe at that time which were biopsied...they were found to be benign. Shortly after that we moved states. Started seeing my current doctor, he did an ultrasound at my first appointment and found nodules were all over my thyroid in just 6 short months...I was sent for a nuclear scan about 2 months later, it came back normal....he ran blood work showing hypo and hyper subscores, but "normal" TSH...but I was suffering exhaustion, hair loss, diahrea, extreme dry skin, intolerance to cold....I would run between menstrual cycles that were disabling in amount and length, to no menstrual cycle for up to a year at a time...I am sick of doctors and test after test...the most recent endo I saw after waiting 9 months to get in to see him, said he felt I shouldnt be on thyroid medication at all and suggested I might have sleep apnea, without even looking at my tests...he ran his own blood work which put my TSh at .376...but Im currently gaining weight and showing all other signs of hypothyroidism, not hyper for the first time in two years...my doctor is furious and wants too send me to another endo, to get my thyroid killed off once and for all so we can control it through medication...I dont know what to do...has anyone else had a constat combination of symptoms for hypo and hyper....any help would e greatly appreciated.

I have Hashimoto's and have moved between hypo and hyper but am hypo most often.  I just received my blood work and was a little surprised.  My TSH was .051 (range .4 - 5), FT4 is 1.48 (range is .87 - 1.31), TPO is 12.9 (range is 0 - 35), and TgAb is 74.4 (range is 0 - 40).  Should I be overly concerned?  My doctor decreased my dosage from 150 mcg Synthroid to 125.  He didn't seem concerned but now I am having an issue with my neck being painful on the left side.

What do you think?

T4 and T3 are "total " tests and are far from telling anythng when it comes to stabilizing the thyroid.

The specific tests are labeled as FT3 ( or Free T3) and FT4 ( Free T4) those are the active hormone that is allowing you to be optimal with thyroid health.

If you have not had antibody testing yet and ablation is the next step already - I fel you need to back away from RAI and get the right tests down first.

Antibodies in Graves and/or Hashimoto are important and you should know this first before you ablate the gland. Especailly in Hashimoto - they attackes can swing from Hypo to hyper often and you can be confused with symptoms. Also with Hashimoto TSH levels most often come out "in normal range" and the patient is not attended to very often.

This could be a temp situation and ablation will permanently kill off your thyroid function.

Thanks for the response. They did do a T4 and it came out "normal"  1.17 ng/dL, but my T3 was <0.03%

as far as the TPOab and TgAb, to my knowledge those have not been run, but will definetly check with my current doctor.

When my thyroid is saying "hyper" I am showing all outward symptoms of hypo and vice versa....I had an old doctor say she hadnt ever seen anything like it, it was if my thyroid ran in reverse of what it should.

I really don't think you are getting the right tests. The TSH - in your caose is not the right evaluation for your thyroid function.

You don't mention any antibody testing either. I would not move forward on any permanent ablation until you know 100% on what's going on.

The TSH can be an asset - but the Free T3 and Free T4 is better to determine the thyroid issue.

You have nodule issues and some Hashimoto patients do have this happen to them. Also the TSH in Hashimoto can be "normal" but the person is miserable with symptoms.

Ask you doctor to run these tests - FT3-FT4 - TPOab - TgAb
