I need information on mulitnodular goiter

Dear Dr.,
  I had an ultrasound on my thyroid and the impression was multinodular goiter and recommended a radionuclide scan to exclude the possibility of a dominant cold nodule. What does that mean? The report also said there are large nodules present in both lobes. I know there are hot and cold nodules. Does cold mean anything bad? If there is any peripheral increased blood flow and measures about 1.7x1.4x1.3cm, what does that mean? In the isthmus, there is a complex area of abnormality with some slight increased blood flow measuring 1.2x0.4x1cm. Thank you for your answer.

Had breast cancer and spread to lymph nodes, did chemo and radiation. Have lymphademia and neuropathy in right arm as well. Pet scan was done at end of treatment and thyroid lit up so then I had a ultrasound of thyroid. It showed I have 4 solid nodules.
Impression:
Multiple solid thyroid nodules bilaterally as described. The appearance is consistent with
mulitnodular goiter. Clinical correlation and continued follow-up is recommended.
I am concerned because there are 4 measuring all different sizes and the past cancer last year. Can you help me?

In your case the RAI scan may not be really necessary, especially if your thyroid hormone levels are normal or low. The present AACE guide recommends biopsy for solid palpable nodules of 1 cm and larger. The nodule with peripheral increased blood flow is probably composed from the dead tissue and most likely is benign.
The area in isthmus needs to be evaluated more closely as it has internal blood flow but the information from the report does not specify if it is discrete nodule or just inflammatory spot.
