TPOab is indicative of an auto-immune problem, Hashimoto's, in this case.
It's also a good reason to ask for other thyroid antibody tests. Your TGab or TRab may be high also.
Looking at all your antibody numbers can give you an idea of how quickly your thyroid will deteriorate over the years. If you have extremely high antibody counts, that means your thyroid is being damaged at a faster rate than if your antibody numbers are fairly low.
A TPOab of 235 is not good news, but it could easily be ten times higher than that, so it all depends on how you want to look at it.
The antibodies have damaged your thyroid and will continue to do so. The question is, will the damage occur faster than the doctors can keep up with it medication-wise.
If your TPOab was 80, and you didn't have other antibodies in high numbers, you would be one of the lucky ones that experience a slow decline of the gland that takes decades and is controlled with medication fairly easily.
If your TPOab was 2,000, you would be on a roller coaster ride for a couple of years, during which time your med dosage would be changing every few months and you would never be right, until your thyroid finally quit working altogether.
As it is, you are in a situation that is unpredictable. You may be able to be brought into balance fairly quickly (a few months) and then your body will start to heal (not your thyroid), and you may need a dosage adjustment every year or two. Or it may be a little harder to control than that.
You should ask for further antibody tests, and an ultrasound of the thyroid would be a good idea. You should also be re-tested in six weeks to see how your TSH looks, and every six weeks until your med dosage is right.
Once your TSH is around 1.0 or so, you should have a TSH and a Free T4 run every three months. That way your doctor can catch any ups or downs of the TSH before it gets extreme, and adjust your meds.
