Cocaine relapse

My fiancé has been hooked on to cocaine for nearly 2 years now, he went to a rehab last month and came back only to get hooked on to it again. I was wondering, if there is any medicine that can help control his craving for the substance. I am really devastated and so is his family. Please guide us on this.

He needs support. There is also celebrate recovery a 12 step program,
Counseling, cognitive behavioral therapy.
It is very, very hard to stay clean without a lot of support.

"I was wondering, if there is any medicine that can help control his craving for the substance."

There are medications being studies right now for cocaine addiction but none have yet to gain FDA approval.

"Nepicastat, a selective DBH-inhibitor that has yet to come to market, is currently under investigation for treatment of cocaine dependence. In preclinical trials, nepicastat has demonstrated an ability to (1) increase synaptic levels of dopamine, (2) decrease brain norepinephrine levels, and (3) block cocaine-induced reinstatement of cocaine seeking in rats without affecting food-primed reinstatement of food seeking [34]. These results suggest similarity between disulfiram and nepicastat in terms of their ability to attenuate responses to drug-related cues and represent an exciting prospect in the pharmacotherapy of cocaine dependence. "

There is currently a clinical trial to see if buproprion (Welbutrin) is effective in treating cocaine abuse.

"The research proposed in this application will determine the initial efficacy, safety and tolerability of a novel drug combination, bupropion and naltrexone, as a pharmacotherapy for cocaine dependence. "

Here's the link to the trial.

Hey there~
Sorry to hear of the relapse. I know this is super hard!!

You've gotten great wisdom already, but I just wanted to add a little...

I don't know the reason - obviously - why he may be reluctant to go to NA... but I can tell you my experience.  I didn't choose to go to NA originally... I attended a couple of NA meetings, but found that the 2 different meetings I went to actually caused some triggers for me. I found the particular meeting FOR ME... was kinda like a place to 'hook up' not to practice recovery.  This is ONE or two meetings... but this was my experience.  What I DID do ... is go to AA meetings!  I just went in...sat... listened... and learned!!  Saved my life.  NOW, after a good bit of time under my belt... I can go to NA meetings with NO issue.  It takes time.

Biggest issue -- HE has to be WILLING. There is no magical pill. WE as addicts have to SEEK out diligently our recovery... run it down like our lives depend on it... because it does.  HE will have to do the hard work.

I encourage you and his family to get support too... nar-anon or al-anon... loving an addict can make us very sick... and we have to work just as hard to be healthy too.
Keep coming back for support!

he could only be there for a month, he was absolutely fine for about 20 days and then he ended up doing it again. He lives with his family. he has been a little reluctant to go for the NA meetings for some weird reason.

He did mention it to me that he is fighting it everyday and sometimes he just can't deal with it.

Hello & welcome. Your fiance needs to use the tools he learned in rehab.
He needs to work a recovery program each day.
Attend meetings, counseling, church,  stay away from the people, places and things  Associated with his use.
His desire to be clean needs to be stronger than his desire to use.
Does he live alone? With you? His family?

He needs to be accountable to loved ones for his time and money.
There e should be boundaries in place and
There should be consequences for his actions.
Do you and his family attend meetings for the family of loved ones?
Al-anon or nar-anon?

No there is no pill to help with cravings.
How long was he in rehab?

Hi Hope, The only medication I know of that is controversial for use in Cocaine user's is vivitrol. This med is mostly used to prevent cravings in opiate addicts. It has it's own list of undesirable side effects but worth looking into. Would he consider going back into rehab for a longer time. Cocaine (from my experience) seems mostly mental rather than a physical dependency and maybe more time with more tools given could help him. Hopefully more will be along with more thoughts and experience on this.

I don't think there is medication to help block cocaine or the urge for it, like they have for alcoholics (antabuse) or a substitute like methadone or subs for opiate abuse. The main thing here is he has to want to quit. He has to hit rock bottom and want to get sober. It is very sad and I feel for you. There is a forum here called Living with an addict! you may want to read through.  I suggest that you and the family withdraw all support and enabling behaviors, and this sadly may mean calling off the wedding for now. You need to protect yourself and your future children from a life surrounded by addiction. This may be a catalyst for change for him. Wishing you blessings and miracles.
