Can Scar tissue/adhesions cause high bilirubin and rhq pain

I have posted here with my symptoms before.....

I had my gallbladder removed laprascopically in Dec 09. I didn't seem to get the "Classic" attacks, more of a constant pain/ache and inflammation. My gallbladder was very inflamed when they removed it.

Since then I have had pain in my upper abdomen which is getting worse.

I have had all the usuall tests done, ct, mri, endescope....etc. All showed normal.
My bloods have showed in the past few time my liver enzymes are slightly high (just above the normal level apparently) and my white blood count is slightly high (also just above the normal), also some level of inflammation.

I have been trialling local anaesthetic injections around the epigastric site scar with the pain management team at my hospital, but so far this hasn't been very effective.

My question is.....is it possible that Adhesions/scar tissue could be the cause of my pain???? I do test positive to the carnetts sign.....hence the thought that it's abdominal wall pain syndrome, but I have noticed deep pain just under my right ribs a lot and my urine is frequently really dark (like apple juice) no matter how much water I drink.

Is it also possible that adhesions/scarring can cause this high bilirubin and pain???

What are your thoughts???? I would really just like an answer to the puzzle, as I am at the point of going back to the surgeons for exploratory surgery......

Thankyou
Sarah

I think I am dealing with scar/adhesions as well. I had a laparotomy back in 09 (sliced from belly button down) to remove my left ovary/complex cyst & a tumor just underneath the aorta (in pelvic area). I was okay for awhile, but for the last year I have had the tearing, pulling, stabbing pains to the left of the belly button all the way up into the left rib cage. I have inquired about the pain to my PCP, but all he says is maybe heartburn (It is not heartburn!). For the last 3 months it has become painful to eat, it just feels like there is a problem of my food going down the intestine on the left side of my abdomain. It is okay if I squeeze it hard or massage it as if i was pushing thru. I do take Advil, not that it helps, but better than nothing. I am down to eating once a day because if I eat lunch I am miserable for the rest of the day & I work. I pretty much eat a light dinner for fear of it becoming to painful. I do normally lay on the couch after about an hour of eating because the pain comes. It is not untolerable pain but very uncomfortable, like maybe air is caught underneath the rib. I can feel the sharp stabbing in the intestine. I do feel okay if I have a BM or if I literally do not eat, but can't do that.

Finally after seeing a surgeon for fistulas he inquired about my low weight, I told him the truth so he actually referred me to a gastro, he set up an ultra sound of the left side & also a small bowel study. When I asked about scar/adhesions he basically said with that you would basically have to live with :( He did say let's see what the tests show first. For two years of complaining my PCP never did anything...except tell me to take OTC for pain....or take Prilosec. I guess I am lucky that I am not throwing up & still able to have a BM. I just get scared to eat for the feeling of being miserable when I do.

We still have not figured out the fistula's yet. I have had 6 peri anal abscesses in the last 2 years & have had a fistulatomy in Feb 2011, but for some reason the issue WILL NOT GO AWAY. I finally begged him, or at least my sister did, to try & figure out WHY I am having such issues with the abscesses & fistulas besides the normal comment I had been getting from another surgeon, "you are just lucky & that is how your body is". I guess in hinde sight I do have a strange body.

I could be wrong or barking up the wrong tree to think that I have issues causing my fistula but I guess I won't know until they test.

Sorry to go on and on...

I assume you are seeing a gastroenterologist about this.  I think they should focus more on your liver.  Also, have you had an ERCP?
