Hi Mike,
Firstly, does your wife suffer from severe headache or migraine? Did they give a specific measurement in the MRI report - as in "cerebellar tonsils extend/protrude [x] mm below the foramen magnum"? If so, then your wife probably has what's known as Arnold Chiari Malformation - you will need to confirm this with your wife's dr / neurologist though. This is a condition present at birth (congenital) where you have like a "hernia" type situation where the base of your brain tissue (cerebellar tonsils) are pushed down and protrude from the opening in the base of the skull (foramen magnum) . Your question doesn't mention whether the tonsil/s actually protrude from the opening in the skull or are just low-lying so I can't be sure. I suffer from this condition and was diagnosed when I had a brain MRI for severe migraines and other symptoms. The condition can cause quite a range of symptoms.

Hopefully this is a good starting point for you. Feel free to send me a note or message or reply here on this board once you've found out a bit more, I've become quite knowledgeable about this condition since I was diagnosed over 10 years ago and have it monitored regularly; eventually I will have to have the decompression surgery on the brain when the migraines etc become unbearable but I'm holding off on that for as long as I can!
Please also bear in mind that some people have this condition without symptoms and it doesn't cause any problems and therefore not much really needs to be done. Once you've been able to read a bit more about it and chat with the dr's you should have a better idea of where your wife is at. Best of luck!
