Chance of HIV due to condom Break

Hello Dr

I had an anal sex with a Sex worker ( a woman ). During the sex, the condom broke. As soon as I realized it, I changed the condom. I think I had anal sex without condom only for about 3 or 5 seconds. Also, I did not ejaculate during condom breakage. However, I wish to know If there is any chance of STDs/HIV due to condom breaking? If the answer is yes, how much is its probability.

I can understand your anxiety but you need to relax. Consider this, for a straight man it is much harder to contract HIV, second assuming she has HIV which i highly doubt what are your chances really? Homosexual Men who receive  anal are high risk yours is minimal, but if you're concerned get tested, it will ease your mind so you can study and move on with your life.

Excuse me for taking your time again. In my country today is the first day of working (contrary to the western countries where monday is the first day of working) and I went to the two different centers of HIV consulting. Unfortunately, I received two different responses:

In the first center, the expert was in the holidays, but the present Dr recommended me to vaccine hepatite B. Also, she wrote me an HIV test. Besides, she gave me the PEP pills free of charge for 3 days. The PEP drugs include ziduvodine (300 mg) and Lamivudine (150 mg) twice per day. She said me that after three days come back to see the expert if it is necessary to continue the drugs for 28 days.

In the next center, however, there was an expert who told me that usage of PEP is not recomended. The expert told me that since you dont know the status of the sex worker and since the chance of unprotected sex is 3 to 1000, so the PEP treatment is not recomended. This answer is close to the canadian_dude09's point of view where he/she believes that it is not a serious problem. While the first approach machs more with LL's point of view which beleives that a risk is a risk.

I found a website http://www.pep.chapsonline.org.uk/pep_basics.htm in which it is recomended to implement PEP treatment for persons whom have had condom failure or anal sex. Thus, I decided to utilize it, at least for three days when I will meet the expert of the first center. Indeed, I have started utilize PEP 36 hours after possible encounter.

In one side, the math tells me there is not a serious problem: the chance of completely unprotected anal sex for circumcision male from a HIV positive is 1 to 600 and the chance of being that sex worker HIV + is 1 to 10 in the rush approximation. Consequently the chance is about 1 to 6000. On the other hand, according to LL, there is a liitle risk which could not be ignored. For a sensational person like me, the second approach outweights the first one.

I am an academic person who studies 8-10 hours a day. But after this accident, my performance of rsearch and study has become zero and I cannot focus on my study even for 15 min. This is due to mood of some persons like me.

Excuse me for this lengthy message, but would you please give me your crtical comments regarding the points have been made above? Are you recommend to continue the PEP traetment?

Based on how anxious you are, I would suggest an std test (including HIV). This will allow you to finally move on with  your life. Also, people who engaging in anal sex with multiple sexual partners (protected or not) should get themselves checked about once every 10 months to a year.

Having said that, the chances of you contracted an std from the encounter you described is extremely low.

Thank you for your comment, I will certainly go to the clinic for the test, but until that time I need to become calm down. Because of this fact I am frequently asking these questions. This is my 4 th sex I have ever had and I am not a professional ( I am a 31 olds from a banned country), so please understand my situation. I just want to know if I am not in a serious problem...

So many thanks for your hopeful as well as helpful response.

As soon as I and the girl heared a " pop " voice due to condom breakage, I pulled off, as a result there was unprotected contact for less than 5 seconds.

However, my concern only relates to the condition of the sex worker. This person ONLY gives the anal sex ( due to some limitations in my country which you may think it is strange) . Although she performs this service only with condom, but I think she has higher chance of HIV due to giving only that service.

What is your opinion with this regard?

Did you read anything at all that LL said?She has already advised you when to test.

most sex workers do not have HIV, especially the ones who use condoms on a regular basis. Sex workers who offer anal sex do have a slightly higher chance of being HIV positive.

Based on what you described above, it is very unlikely that you contracted HIV. Even completely unprotected, the chance of a male contracting HIV from an HIV positive female via anal sex is less than 1%. Your brief time of exposure essentially reduce the risk to virtually zero.

Other STDs are very unlikely as well. If you really want to get some tests to calm yourself down, chlamydia and gonorrhoea tests could be done although I would assume the results would be negative. I do not recommend HSV tests unless symptoms such as lesion and blisters appear.  

If any of the tests came back positive, I would bet it was from an encounter in the past.

Having said all this, if you are a person who have multiple sexual partners (sex workers or not), you should get tested for STDs (including HIV) about once a year.

a risk is a risk...but due to the MINIMAL time of exposure, i wouldn't call it a high risk.  couple that with the fact that, contrary to "popular belief", most sex workers don't have hiv.  the use condoms to protect THEMSELVES from YOU...not the other way around.

either a duo test at 4 weeks or an antibody test at 6 weeks, will give you a very good indication of your status.  those results are unlikely to change when you get your 3 months conclusive result.
