Is the posted EF40% a new condition?  Is the MID ANTERO SEDTUM APICAL IVS, MID APICAL ANTERIOR WALL TRUE APEX HYPOKINETIC a recent finding? If this is a recent condition then you would need immediate medical treatment.  The treatment would be reducing the heart's workload, and in ischemic conditions that is usually related occluded vessels, and medication to dilate coronary vessels, etc. would be the protocol.

Your echo indicates there has been ischemic damaged to heart cells and the ischemia has impaired heart wall impairment (hypokinesis).  The hypokinesis is likely to be the underlying cause for the low EF40%...normal is 55 to 75% and represents the amount of oxygenated blood pumped with each heartbeat.

Q: Diastolic dysfunction : yes

>>>>DD is a condtition that relates to the filling phase of the heart cycle.
