First of all, if you are currently having chest pain shortness of breath, etc., have someone drive you to the ER or call 9-11.
Secondly, you need to find a different primary care physician and report the one you've got to the medical board for refusing to refer you given your holter monitor results and symptoms.
Thirdly, a decent primary care physician should be able to order the diagnostic testing, such as an echocardiogram to rule out congestive heart failure. And you need to get referred to an electrophysiologist about the arrhythmia you are experiencing.
It might be that you have dysautonomia and an electrophysiologist who conducts tilt table testing should be able to work on determining if you do. Fourthly, since you are having bad sinus congestion and dizziness, you should get referred to an ear, nose and throat specialist and see if you can get some help since evidently your doctor hasn't helped you with this issue either?
Fifthly, ask to get your ferritin checked, which are your iron stores, because this is the best indicator, according to my doctor, for iron in the body. If you have low iron, it can cause extreme fatigue, dizziness, rapid heart rate and chest pain.
A normal CBC (complete blood count) does not necessarily mean your iron is not low. I had real symptoms of iron deficiency and a normal CBC in the past myself. When my doctor at that time dug a little deeper with testing, he found out my iron was low.
