Tuberculosis, Pseudomonas Luteola, 2mm Granuloma

I have a 2mm Granuloma in my lower right lobe lung.  Is this from TB or pseudomonas luteola?  I haven't been able to exhale for years and have terrible asthma and congestion and am disabled from brain fog

Well that is what the Lymes disease doctors use for two months. They use other drugs for the co-infections. I can give you some links if you want them. This has some more information. In the UK they treat differently.  It does depend if you are chronic or not and other things. See links. The second one talks about treatment. I cut part of one of the treatment links below.

Clinicians should prescribe amoxicillin, cefuroxime or doxycycline as first-line agents for the treatment of EM. Azithromycin is also an acceptable agent, particularly in Europe, where trials demonstrated it either outperformed or was as effective as the other first-line agents [46–49]. Initial antibiotic therapy should employ 4–6 weeks of amoxicillin 1500–2000 mg daily in divided doses, cefuroxime 500 mg twice daily or doxycycline 100 mg twice daily or a minimum of 21 days of azithromycin 250–500 mg daily. Pediatric dosing for the individual agents is as follows: amoxicillin 50 mg/kg/day in three divided doses, with a maximal daily dose of 1500 mg; cefuroxime 20–30 mg/kg/day in two divided doses, with a maximal daily dose of 1000 mg and azithromycin 10 mg/kg on day 1 then 5–10 mg/kg daily, with a maximal daily dose of 500 mg. For children 8 years and older, doxycycline is an additional option. Doxycycline is dosed at 4 mg/kg/day in two divided doses, with a maximal daily dose of 200 mg. Higher daily doses of the individual agents may be appropriate in adolescents.

Selection of the antibiotic agent and dose for an individual patient should take several factors into account. In the absence of contraindications, doxycycline is preferred when concomitant Anaplasma or Ehrlichia infections are possibilities. Other considerations include the duration [27,32,50] and severity [50–53] of symptoms, medication tolerability, patient age, pregnancy status, co-morbidities, recent or current corticosteroid use [54,55] cost, the need for lifestyle adjustments to accommodate certain antibiotics and patient preferences. Variations in patient-specific details and the limitations of the evidence imply that clinicians may, in a variety of circumstances, need to select therapeutic regimens utilizing higher doses, longer durations or combinations of first-line agents (Recommendation, very low-quality evidence).

Another problem is that Lyme hides inside the lymph nodes.

but doxycycline cannot be good it is only a bacteriostatic drug and doesn't work against biofilms

is there a treatment or drug that helps hypoxia to the brain like citicholine or piracetam or the newer racetams?

they won't give me an appointment at Columbia and the other doctor I saw was terrible and didn't run a blood test

I'm having with hypoxia and dangerously low oxygen and it seems to cause high glutamate levels in my brain which causes severe fogginess.
rocephin is supposed to increase expression of GLT-1 glutamate transporters.  I took 10 days of it and my heart problems seemed to be cured and I had a reduction in post sleep fogginess.  No more very uncomfortable heart racing and tachycardia of 125 and chest pains.

That is interesting. They are often ahead of the game. I hope it works. That is awful regarding Azithromycin. I have seen some bad reactions from it..

in Europe some LLMDs are going with 16 weeks of rocephin and tinidazole, lymecycline and azithromycin  

I would stay away from azithromycin, it is a dangerous drug, I tried the weekly asthma cure and I cannot even exhale any more 2 months after trying it

Ceftriaxone (Rocephin), is a broader spectrum drug but the standard treatment for Lyme is Doxycyclin.  But I'm glad you have an appointment at Columbia. I hope it goes well. Let me know what you find out.

rocephin is a better drug than doxycycline.  I took 10 days of rocephin and it helped my fog and heart problems and then I started to have constant neck pain like from infection possibly
my internist was really nasty so I don't know if she'll write the referral but at least I got an appointment at Columbia

Yes it is true you can get on disability in the US. It is not a lot of money but it helps. Doxycycline is the main drug for lymes. They also have some herbal treatments but those are expensive. I know they have cut my friend off of food stamps as well or down to like $12.00 a month which is nothing and she has chronic lyme and can't take care of herself. She does get a nurse to come in and help her though and she doesn't have to pay for it.  You really need to get tested for lyme and get a western blot too.

It's really difficult to travel to see an infectious disease doctor and pay for it.  I'm really really afraid that I have Lyme's with the hearing loss, humming, blurry vision, seizures, sleep disorders, facial palsy, knee pain, fatigue, memory problems
Yes it's nice to have food stamps but they just cut mine off for no reason.  You can also get disability in the US

Hi, sorry you are going through all this. It is possible the cyst on the Clivus could be causing some problems. This is a technical article. But it shows that they can do a local anesthetic in some cases to drain the cyst.

Can you get Obamacare or the ACA? or are you on Medical or Medicaid? I don't know what exactly happpened to those programs now that they have the ACA.  I know they cut food stamps. It is terrible. I do understand how hard it is having been on it in the past.

Do you have any family or friends that can assist you in getting paperwork done or taking you places? Or maybe getting a social worker to help. I know they are not very helpful at the Medical site but maybe through a local free clinic they can give you advice as to how to sort things out?

It is important for you to try to get someone on your case.

I also had neurofibromas in my colon and a neurenteric cyst eroding my clivus.  Could any of this cause my terrible fogginess and sleep disorders?  I on food stamps and barely surviving

I am so  sorry for everything you have gone through. How long ago do you think you were bitten by the ticks? If you are lyme positive you should follow up with a western blot to check if the bands are correct for lyme. If so They they treat you with  Doxycycline for about two months and see how you do. You could have some of the co-infections that go with a tick bite such as Erlichia, Bartonella, and babesiosis.

If you have babesiosis :babesiosis is treated with a combination of two types of anti-parasite drugs, atovaquone (Mepron, Malarone) plus an erythromycin-type drug (azithromycin, clarithromycin, or telithromycin). Long-standing infections may need to be treated for several months, and relapses sometimes occur and must be retreated.

So I would get tested for these co-infections and not kill yourself with all the antibiotics if you don't have these organisms.

Do you have a laboratory where they can test for these? A reference laboratory can do this.

Yes some antibiotics cause otoxoicity. I hope it is not permanent.

If you were TB skin test negative then you may not have TB. The nodule may be caused by something else. I would ask the doctor what other possibilities it could be. Also a fungus ball is something to think about such as Aspergilloma. I hope not. But if so you would need an antibiotic plus steroid. You need to get diagnosed though. As if you gave yourself the wrong thing you may cause yourself to worse.

The bumps on the leg could be staph infection or some type of infection. Can you get them cultured?

You really need to see a good physician about the nodule and weight loss plus what the nodule on the lung is. They could do a biopsy and culture the leg sores with pus. Mention the lyme results and also that you want to be checked for co-infections and maybe follow up with a western blot for lyme.. Also mention that the antibiotics made you feel better. Go from there.

I had bilateral raised red bumps on both my legs which filled with pus but no leg swelling.  
The pseudomonas culture showed sensitivity to gentamicin according to the doctor so I also tried 10 days of gentamicin 80mg/day 1 ampoule a day but I couldn't exhale still- I guess I've lost my hearing forever from ototoxicity

I thought I had a stroke, the hypoxic damage must be terrible after years and my face is palsy and my eye went blind from muscle weakness.  
Even taking prednisone at 60mg I can't exhale or with daily dulera/singulair.  I'm trying to get tracheosotmy for the 2nd time because the surgeon put me on a shiley 6.
I really loved doing science and theology but I'm so brain damaged after being attacked I can't pronounce words and I'm losing my hearing too.

Yes I've been bitten by ticks.  anyway I'm going to try a GAPS, no gluten, no sugar diet to try to reduce neuro inflammation with piracetam and high dose b12 and amaybe more rocephin.  My Lyme Iga/Igm was 0.11 with 0.8 as positive, but I've heard it's not a reliable test.

I've tried TB skin tests but they come back negative.  I'd like to just try the TB antibiotics to see if they improve my breathing and brain damage?

I tried 10 days of rocephin with some clarithromycin and a shot of gentamicin and I had more energy and I could wake up without the disabling brain fog and neuro inflammation on rocephin.  But I still can't exhale and am brain damaged.

My tachyhardia went down from 125 to 65 and my heart rhythms were finally comfortably working for the first time in years.

sorry I had another though before I run off. I still think you should be tested for TB. You still have those symptoms in addition to the Pseudomonas.

p.s. the pseudomonas may be a secondary infection in the lung.

However, this was a rare case of P. luteola from a tick bite that has some of your symptoms.

Do you or did you have leg swelling? Did you get a complete blood count done? If not you should have that done as well. The blood cultures as urine culture as it mentions in the article.

If positive then you can take the antibiotics suggested in the article.

Hi and so sorry you have all these problems. If you haven't had these things done, have you been vaccinated with the BCG TB vaccine? If not I would get a quantiferon gold TB skin test. That would tell you if you were exposed to TB if so then you need a sputum culture for TB. You have several symptoms of TB. The fever, granuloma, and bloody sputum.  The knee pain and neck swelling may be something else.

Then I would also get checked for malaria and other parasites and Leptospirosis,  meningitis, etc.

If they have a infectious disease doctor there I would see that type of doctor.

Were you bitten by a tick? that is how you get Lymes and the co-infections of lyme. You can get tested for those if you like.  But it doesn't usually cause the granuloma you have or bloody sputum.

Also, did you swim in fresh water? There are other parasites that can affect the eyes if you did. But if you are sure it is glaucoma then be sure to keep getting tested and take the eye drops to keep the pressure down.

There is a parasite schistosomiasis that can affect the liver. So maybe a stool sample for parasites as well. A couple of different days.

Yes I've had bloody sputum and low grade fevers and chills and knee pain.  I can't exhale at all and would for my whole life as a child wake up hyperventilating with chest congestion.
Couldn't this also be Lyme's or Bartonella with the granuloma and neuro inflammation?  The kneck swelling is cutting off circulation to my brain and killing me.  I'm also going blind from glaucoma and had serious liver damage, who knows where else these granulomas are I have kidney cysts

I really love microbiology and was 4.0 in physics also; I dreamed of becoming a hospital microbiologist.

Thanks so much for your help!!!  I am slowly dying now, I was teaching on faculty in Indonesia, I was bitten by a mosquito on my uncovered hand and had terrible migraines and collapsed from weakness and had diarrhea and nausea and vomitting for a week. I have terrible brain damage, severe fogginess, sleep disorders (seizures, hypnic jerks or twitching).  My mother had TB when I was a child.

I'm a theologian and mathematics student until I was disabled and crippled by these diseases.  I tested positive for pseudomonas luteola and staphylococcus maybe MRSI on a semen culture and severe prostate infection at the urologists (I'm a theologian).

Hi and sorry you have been going through all this. I am not a doctor but I am a microbiologist and can try to give you some advise.

What country do you live in? Some countries have more TB exposure than others.

Why would you think you have Pseudomonas luteola specifically? There are many things that can infect the lungs.

So, do  you have a low grade fever or any fever? Do you have any cough, night sweats, fatigue?

Have you had the BCG TB vaccine? Has any sputum culture been done?
