Doctor doesn't know what the problem is

ABout 4 years ago, I developed ice pick like stabs in my head through the roof of my mouth to the top of my head. I also had stabs in my right eye. My right nostril constantly dripped. i was put on Gabapentin (1500 mg) and did Ok for about 2 years. (the stabbing was not as painful/frequent)  about 9 months ago I started to get worse. more painful and frequent stabbing pains so my neurologist switched me to Lyrica (200 mg a day)  Initialli it seemed to help but I am now experiencing stabbing pains at least 3 times a day which leaves me with throbbing in my head that leaves me incapable of functioning. it has spread into my throat (the stabs) as well as the left side of my mouth. My doctor has me on steroids for a week to see if it will help. it is not. My intial diagnosis was migraines but didn't quite fit and trigeminal neuralgia which didn't quite fit. I have had 2 MRI's and a CT scan as well as lots of blood work which all came back normal. At my last visit my doctor said he doesn't know what is causing the pain above my mouth and through my head. i go to a large teaching faciltiy. Should I get a second opinion?

I just read this and had to kinda relate to it a little. My brother had these and they were called literally Ice Pick headaches. Very fast onset and happened daily for a few days before he decided to seek a little help. I think all he did was take a mild NSAID for a few days and they were gone. I would say in his case it might be slightly stress related. But, if its something aleve or an anti-inflamatory med will prevent or help reduce the attacks, it might be worth looking into for a full time med on a daily basis. Mobic is something we use takin with food seems to have little side effects if any.

How are you? If the medications are not helping and imaging/blood tests done were normal, have this evaluated further for proper diagnosis. Although prolonged nasal discharge may indicate allergies or sinus issues. You may also benefit from an ENT consultation. To ease any doubts, a second opinion may also help.
