Yes - I know - because such male urologists instead of viewing the patient from a purely clinical perspective, and compassion, are externalising their own fears of losing their "manhood" - so really they are failing the patient due to their own fears and prejudices - with respect to any urologist here who may be an exception.
My testes have been diagnosed as not producing any testosterone or sperm, I am already on HRT, which is excellent, they are atrophied and retracted, causing much pain, are an increased cancer risk as you say - so removing them should be an absolute certainty.
Well - since the surgery does not bother me and I won't miss them, I have already commenced the process of seeking a urologist to remove them.

It's not just male urologists. 99&400% of men think that is a fate worse than death.
Understand that I'm not a doctor, but I believe retracted testicles are somewhat more prone to developing cancer.
I do wish you luck, and also wish one of the urologists on this board would chime in with some additional information and an opinion.

That definitely has to be the route to take. Since my testes are totally dysfunctional and atrophied and already on HRT for the last year, nothing to lose and everything to gain.
Just a matter of finding a urologist to do it. I can only assume that male urologists are prejudiced against orchiectomy because of their own deep seated fears or feelings about it as a male, yet women are routinely given full hysterectomies and stop producing hormones anyway at menopause.

Mine were removed through inguinal incisions, and all the veins and spermatic cords were taken. The end result is I have been pain free down there for many years. At least for me it was worth the fight with my urologist. The pain of surgery receded quickly and I'm now a productive (no pun intended) human being again.

The pain I suffer is also debilitating, but appears to originate further up than the testicles in/along the cord itself, but appears as pain in the testicles.
I believe the medical term is Orchialgia.
Since I am already on HRT - 3 monthly injections - and my testicles have atrophied and retracted and diagnosed as totally dysfunctional, and since I have grown children, then having them removed would be an easy decision. For some reason though, doctors seem to prefer to treat the pain - not effectively, than the origin.
Also, since the pain definitely extends to the cords as well, having the testes removed alone may not resolve the pain.

Back in the eighties I suffered from the pain of varicoceles on both sides so I can empathize. That said, you may not be amenable to the choice I made to have both testicles removed. Had one hell of a time finding a urologist who was compassionate enough to do the surgery. I found the pain so debilitating I could not do a day's work.
I, too, was on HRT for a long time afterward, but if you are already on it you've a head start. It does indeed work, and thirty years later I still have a satisfactory sex life.
End result is after my surgery I returned to being physically able to function normally and once again enjoy life. I do not regret my choice in the least.
