Testicle Pain after urination

I have been having  bouts of prostatitis for the last 7 months however lately when I urinate and push a little bit I get a fair bit of achey pain in my spermatic cord area and base of penis.  This is also amplified when I have a bit of an errection.   Embaressing, but does anyone have any thoughts?  It stays sore for hours sometimes  I am on anti inflamitories and alpha blockers.

Hi Doctor, I have had the DRE at least 4 times!  when it is bad, the prostate is sore as can be and I felt burning in my testicles after.  Sometimes the doctor says it is 'boggy' but not that much pain and then a few hours later I can be in much pain as its been irritated.   I am currently taking the flomax now and a once a day anti inflammitory.  I should mention I am only 34 and before this, healthy too.  Some forums have reccomended something called 'PEENUTS'  Have you ever heard of this?  Or am I being taken for a ride?  They claim that it has fantastic success for people in my position.  http://www.peenuts.com/  I am very skepitcal because I am already taking Relexin and Saw Palmetto with no results really.  Is this something that can go away in time?  Its been 7 months now and it seems I can only allivate the symptoms and not get rid of this.  

The only thing that has really strange is that when I push hard at the end of urination my spermatic cord area can become very sore for hours if I don't pee slowly.   Also for some reason the last few days I am having problems figuring out if my bladder is empty or not.  I know when I have to go, but I find it hard to know if its empty without the pushing.

Does this shed any light on anything?

Hi,
     Thanks for writing in. I think it is the enlargement of your prostrate which is causing the obstructive symptoms leading to urinary tract infection.
I would suggest that you get a DRE for the prostrate, cystoscopy and voiding cysto urethrogram. Flomax is used in the treatment of BPH, so it must probably has been prescribed for that cause.
Also discuss with your urologist regarding options of treatment for BPH. If conservative measures fail, surgical treatment with TURP will help.

Thanks for the fast answer doctor.  I have an ultrasound that showed it was not epiditimitis.  Doctor also did a culture with no results.  My prostate can suddenly flare up and feel large, warm and swollen.  Its seems that the testicle pain which is in the base of the penis and down the spermatic cord a bit is always happening in sync with the prostate pain.  The doctor said to take the flomax/alpha blockers I think because he is out of options.  Been taking that and a once a day anti inflammitory.  I am 34 and was healthy before this so I Dont think its BPH.  I have seen a urologist and he is a little baffled.  He mentioned something about the possiblilty of a cystoscope but my family doctor thinks its a waste of time.   I was on cipro, apoflox on and off  and then vibratabs for 6 weeks for a total of around 11 weeks of antibiotics with no change.  Maybe some kind of chronic non bacterial thing?  I have been off spicy foods, caffine, alcohol, etc.  Also taking saw palmetto and bee pollen, and all that other herbal junk that I never believed in anyway but I am desperate to get past this.

Maybe this information can help you, as you can imagine I am very frustrated with no relief in sight.

    How are you? Recurrent bouts of prostatitis or any urinary tract infection i.e., infection of any part of the urinary tract can affect the adjacent parts easily. As you already mentioned that you had recurrent prostatitis, there could be a chance of orchitis causing pain in the testicles and the cord. Also epididymitis may sometimes mimic this pain.
Pain at the base of the penis could be due to balanitis. I would suggest that you get a urine analysis with culture done. Also a physical examination with a urologist would be prefferable.
What is the indication for using alpha blockers? They can be used for BPH. Sometimes BPH itself can cause retention of urine and infection there by in the tract.
Consult your doctor and discuss the possibilities.
