Varicocele

Hello

I am an almost 25-year-old male from Poland. I have some problems I would like to ask about.

So, first of all, I have varicocele on my left testicle. I've had it for a very long time. When I was about 20 I got a surgery for it but unfortunately it didn't help. I know I should have done something about it then but I just hoped the varicocele would eventually disappear. It didn't. Now I am thinking about getting another surgery. But I'm not sure. Do I need to go to a urologist again, have ultrasonography performed and so? I remember that last time I also needed to be vaccinated for hepatitis B (if I remember correctly). So I waited over a year for the operation. And if the first operation didn't succeed, is it possible that another one will succeed?

I also have some problem related to the ejaculation. I don't have a sexual partner but I get nocturnal emissions from time to time. I sometimes masturbate (very rarely). Each time after the ejaculation I have some strange feeling at the tip of my penis. I would describe it as some kind of sensitiveness. However, it doesn't really hurt, burn or anything. It just feels like I needed to pull my underwear away (and that's often what I do). It's like the tip of my penis was more sensitive to being touched by the underwear. I know that after ejaculation it's supposed to be sensitive but why does it last so long? It always lasts for at least one day. Could it be related to my varicocele? Well, it doesn't seem serious at all as I didn't notice anything other that would be suspicious. But I worry about it a little.

I'll appreciate any help.

Hi! Yes, STD is ruled out. A repeat ultrasound of scrotum should be done. It can also be that you have a hydrocele or maybe infection of testis and epididymis. One simple explanation is sexual arousal that does not end in complete ejaculation can cause congestion in the sperm carrying tubes. This causes a dull aching sensation in the testicle also called ‘blue balls’. Masturbation helps relieve this pain. So you can try this. You are seeing your doctor so discuss with him as he can judge this best. Take care!

Thank you for answering. I am going to see a urologist anyway. But I would like to ask some more questions now.
If I have never had a sexual partner, can STD be completely ruled out? As for the varicocele, I feel some swelling from time to time in the left testicle. However, after an ejaculation I feel something different than that. I called it a sensitiveness but maybe I should have said that it's a tightness. I don't feel any pain when I touch my penis. It feels more like my underwear was too tight. When I take my underwear off, that feeling goes away but it returns when I put it on again. I feel that it comes from the tip of my penis but I'm not sure. It's not well localised, you know. Sometimes (usually a day after the ejaculation) I feel such tightness also in the testicles, the groin area. And again it's only felt when I have my underwear on. It happens only after an ejaculation and it lasts quite long (up to three days).

Hi
Welcome to the MedHelp forum!
If the varicocele is asymptomatic and infertility is not an issue, no treatment is required. Mild discomfort can be managed by wearing an athletic supporter or snug-fitting underwear during strenuous activity or exercise. However if the person wants to be a father in future (which naturally you may eventually), it is better to get it operated. Varicoceles are known to cause infertility as they affect sperm production. Usually it will not effect ejaculation unless complications arise. Treatment options are surgical ligation, embolization and laparoscopic approach. Also you would need to maintain a diet rich in fibers and multivitamins and low on refined carbohydrates to bring strength to the surgical site and also to avoid constipation and straining. This will prevent recurrence too. Since you have developed it again these factors of recurrence will have to be taken care of. Yes, a repeat ultrasound would be needed. Indications for repairing a varicocele again in adolescence usually includes progressive testicular atrophy (some atrophy is expected), pain or abnormal semen analysis results.
Since you have a sensitive tip of penis get a complete STD panel done if you had a sexual partner in past. Sensitive tip can also be due to balanitis, prostatitis and eipdidymitis. Please consult an urologist and discuss your case. Take care!
