One simple explanation is sexual arousal that does not end in complete ejaculation can cause congestion in the sperm carrying tubes. This causes a dull aching sensation in the testicle also called ‘blue balls’. Masturbation helps relieve this pain. So you can try this. If I have never had a sexual partner, can STD be completely ruled out? As for the varicocele, I feel some swelling from time to time in the left testicle. However, after an ejaculation I feel something different than that. I called it a sensitiveness but maybe I should have said that it's a tightness. I don't feel any pain when I touch my penis. It feels more like my underwear was too tight. When I take my underwear off, that feeling goes away but it returns when I put it on again. I feel that it comes from the tip of my penis but I'm not sure. It's not well localised, you know. Sometimes (usually a day after the ejaculation) I feel such tightness also in the testicles, the groin area. And again it's only felt when I have my underwear on. It happens only after an ejaculation and it lasts quite long (up to three days).

If the varicocele is asymptomatic and infertility is not an issue, no treatment is required. Mild discomfort can be managed by wearing an athletic supporter or snug-fitting underwear during strenuous activity or exercise. However if the person wants to be a father in future (which naturally you may eventually), it is better to get it operated. Varicoceles are known to cause infertility as they affect sperm production. Usually it will not effect ejaculation unless complications arise. Treatment options are surgical ligation, embolization and laparoscopic approach. Also you would need to maintain a diet rich in fibers and multivitamins and low on refined carbohydrates to bring strength to the surgical site and also to avoid constipation and straining. This will prevent recurrence too. Since you have developed it again these factors of recurrence will have to be taken care of. Yes, a repeat ultrasound would be needed. Indications for repairing a varicocele again in adolescence usually includes progressive testicular atrophy (some atrophy is expected), pain or abnormal semen analysis results.
Since you have a sensitive tip of penis get a complete STD panel done if you had a sexual partner in past. Sensitive tip can also be due to balanitis, prostatitis and eipdidymitis. Please consult an urologist and discuss your case. Take care!
