MEDICAL PROFESSIONAL
Hello,
For such scratched skin, Wash the areas with fresh water. Do not use any cosmetic products at the sites. You can apply some calamine lotion at the rash as it will help in soothing the skin. Also wear cotton undergarments.Application of mild corticosteroid cream and oral antiinflammatories like ibuprofen/acetaminophen are also helpful. If the symptoms are severe then it will be best to consult a dermatologist.

It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. For exact diagnosis, you are requested to consult your doctor. I sincerely hope that helps. Take care and please do keep me posted on how you are doing.

Typically these areas heal quickly, with just a scratch.  A soothing bath at night before bed is good, don't use soaps or salts in the water.

I am seriously suggesting you NOT use toilet paper
Instead use only baby wipes til healed.  I NEVER use toilet paper.  The rectum is much cleaner with wet baby wipes.  This is a gentler and cleaner process.

that area is called your perenium and can be painful when urinating.  The wet wipes will dilute the urine some and sooth the wiping process.  I can not think of a cream to use for that area that would be OTC (over the counter), sorry.

warm water baths, no soap and wet wipes are all I can offer at this time.  Sorry I'm not more of a help.
