Hi, welcome to the forum, multiple lesions suggests that there is wide involvement of the disease and it has involved particular areas. The diseases which involves in such fashion are tuberculosis, viral pneumonia, chronic inflammatory disorder like SLE, sarcoidosis, Rheumatoid arthritis (These disorders are known to affect vital organs like lungs, kidney, heart etc. Interstitial lung disease is long term effect of such disorder and significantly reduces the oxygen saturation.) and the lung cancer.

Metastatic lesions are the one which spreads to an unrelated tissue (from else where to lungs). This can be a disease or it can be a cancer. You have not cleared your primary disease.

You need to undergo certain serological tests to rule out such disease (RA factor, ANA titer, sDNA etc.). Prompt treatment is quite necessary before the advent of advanced fibrosis which is a irreversible damage to lung tissue.

Such lung damage is highly likely to respond to glucocorticoids and you are already on it.
I suggest you to consult respiratory physician. Take care and regards.
