Death by colonoscopy

What is the average yearly death rate for persons while they are undergoing a colonoscopy?

Type in "colonoscopy mortality rate" into google and you will find several articles that elaborate on this.  The statistics probably require sub-divisions such as:-

- outright death during the procedure
- when death occurs after perforation during the procedure
- when death occurs to patients who are already seriously ill/hospitalised
etc etc
