Self-gagging when independent bowel movement but not when due to fleet enema. What would cause this?

I know a person who is developmentally disabled and can not communicate. He is 52 years old. He gags himself on the days that he has a bowel movement without the use of a fleet, but not when he has one as a result of a fleet. I don't believe it is for attention, etc. Is there a medical reason that we should check into?
