Putty like stool

I have been suffering from hepatitis b(could not be confirmed) and liver cirrhosis for a long time. For the last three days, I am having putty like stool (stool color is normal) which is causing much discomfort. I also have, most the of the time, constipation. The last portion of the stool remain unclear(not evacuated). This also causes much discomfort.
Please let me know what to do in this situation.
