Dear Dr. Schiano

I am 43 yr old female with hashimoto's disease and on synthroid 50mcg. I had posted 3 months ago in regards to testing positive to AMA antibodies but liver panel normal, however I was tested again for the same tests and my ALT came back 54 (5-45) higher than normal for the first time and my TSH was 7.8 unlike other times always in normal range. I have been taking 50,000 vitamin D3 twice a week for the last 2 months and took some advils for cramps for couple of days before the test. I'm not overweight at all, and don't drink, however the last couple of months, I did eat out a lot having had out of towners visiting us and probably consumed much more fat than my daily routine. My question is: Should I be concerned about my ALT level or could it be that the high dosage of vitamin D caused that, since my numbers were in range before I took them. Or could my sluggish thyroid cause a slight elevation in ALT?

the slightly elevated AMA is not due to the vit D and in theory could be due to the thyroid disorder.  NSAIDS can make the liver tests rise also.  You should follow the titer of the AMA and see if it remains (+); PBC is associated with Hashimoto's.
