copy of neuro report

hi folks
as you know i went to see the new neuro the other day and he was great!!!
today i hve recieved a copy of his report he has sent to my gp following his examination of me
its a bit long winded but the main bits are as follows.

I think your investigations  so far have documented lumber spine degenerative disease with some minor evidence of minor nerve root impingment. But nothing that would clearly explain either his symptoms or his clinical signs.
he noted some bladder instabillity over recent times as well as his walking difficulties and as you have pointed out to hime on examination he clearly has abnormal neurology.
i was unconvinced ther was anything clearly abnormal in the cranial nerves. but in the upper limbs tone was increased on the lefrt, more so than the right, power was well preserved, reflexes were pathalogically brisk on the left with possitive hoffman's sign, sensation was remarkable only for diminished pinprick at the finger tips and vibration was normal. in the lower limbs ther was pyramidal increase in tone of the left leg, easy elicitible clonus at both ankles, power was well maintained on the right hand side. in the left there was pyramidal weakness.
reflexes were pathalogically brisk.

i am cocerned weather or not we may be seeing either central demyelination or possible sructural cervical spine disease.
but with obviously without having the results of his imaging to hand it is difficult to comment at thuis point.
i have asked my secretary to get hold of the original images for review.
i will be requesting repeat MRI brain and cervical and possible csf examination.

I think it's great that you are finally getting somewhere! Here's hoping that from now on you'll have smooth sailing.

Notice you have bumped this a few times. I'm not sure what further comments you are seeking. We too await the results of your further testing, at which time maybe new thoughts will come to mind.

It sounds like your situation is very similar to mine, however our doctors are taking very different approaches.

I have spinal cord impingement in the C spine (C4-C5, C5-C6) and DDD, so the neuro concludes that this is the cause of the majority of my sx. I had problems in my lumbar spin years ago without impingement, but recently have had severe pain in the lumbar area and will have an MRI done of the lumbar spine next Wednesday.

She then wants me to go to a rhuematologist, probably to find out why arthritis is affecting my back and treatment to stop its progression.

No plan for my brain.I also have sx that no way fit into spinal cord damage.I have abnormal brain MRIs with 14+ lesions and sx that cannot be attributed to spinal cord damage. I'm getting all kinds of hemming and hawing over the brain lesions and sx from a neuro who specializes in demyleminating disease. She tells me location doesn't really matter. OK, so I have a juxtacortical lesions, multiple lesions in the periventricular region and 2 lesions in the corpus callosum.

I agree that the two sets of clinical data can make it hard to determine what is causing which symptoms. I applaud your doctor for "using his mind" to quote Laura and recognizing that there are two very distinct sets of symptoms and therefore probable causes.

Keep us posted. I'm going for consultation for as a candidate for back surgery (that one scares me) and will be interested to find out what they recommend for you.

Well Bairdy, my thoughts are FINALLY!!! You have connected with a doctor who listened, observed and is ready to work on your behalf to get answers.

More than once I have said that I think as we age we are going to get signs of disc/spine deterioration.  It just makes sense that the impact over time is going to cause changes - minor for some and major for others.  That the neuro picked up on the spine problems being possibly separate from the neurological issues is an excellent sign that he was willing to use his brain.

Hurrah for new tests - perhaps they will yield the finals clues for this doctor.  I hope they can get you scheduled soon.
