Hello,
I am a 40 year old female with Arnold Chiari Malformation/Syringomelia.  I underwent decompression/partial laminectomy C1/duraplasty brain surgery Dec 30 last year.  The surgery was a success as far as most of my symtoms (symptoms) were relieved and my flow study showed normal flow and no impedements.
Here I am 7 months later with all of my symtoms (symptoms) returned and a few new ones to add to it.  My current worst symptoms are:  excrutiating headaches, imbalance, nauseau, and tingling/heat/pain and numbness in my hands, feet and legs.  
I had a repeat MRI/flow study done.  It showed restricted fow.  They believe that it is caused by scar tissue.  Could scar tissue form that quickly? Could it have been, instead, a bleed after surgery,and the collected blood?  I was informed that removal of the scar tissue was not the best option, as it would regrow.  I have a spinal tap next week to see the level of my pressure so as to decide if a shunt could be an option.

Sorry to hear about your return of symptoms.  We have seen a few patients who underwent a similar operation to yours who had recurrent symptoms.  Sometimes it is difficult to know why.  Without seeing the scans I can tell you about the scar tissue.  Scar tissue is the result of the healing process and would begin to occur very soon after surgery and continue for a short time.  It does not seem reasonable to me that it was caused by a bleed, since a bleed would have happened very close to the actual operation and you had complete remission of your symptoms.  See what the LP pressure shows.  If suggestive of increased ICP than you may need a shunt.  They also may try diamox.
