Kind of an off hand question, but related to chronic pain. I receive disability benefits. I suffer from chronic pain resulting from brain surgery in 2007. The condition is called central pain syndrome, really refractory to treatments. Very frustrating. Anyway, my disability case is up for review. Thankfully, I am having not a full blown review, but an update report instead, the SSA- 455-OCR-SM form. Here's the deal: I decided to change part of an answer on my report a day after I finished it. I had made a copy of the form before I filled it out, back up in case I wished to changed something. Problem was, I had screwed up slightly on the photocopy I made without realizing it when I made the actual copy, (two numbers on the side got cut off,) so I have no backup. I called the SSA 800 number on the instruction form, and they are sending me a second form (hopefully soon,) but this form will go to the Jamaica office in New York, versus the usual processing center in Wilkes Barre, PA, where the first one was destined to go. I think this is the general way they do things when someone does not respond to the initial request. But question is, is the second copy which is sent to the Jamaica office also an OCR (optical character reader) or is it an old fashioned "someone will read it" form? I just hate to have the little thought of another complete review in the back of my mind. I have nothing to hide, but it's such a tiring process....two has been plenty!
