lost singing voice

Hi, I was a professional singer guitarist for over 20 years but I have had to give it up because of a voice/muscle tension problem. . It started over 3 years ago when going towards the microphone my chest and neck muscles would tense, making it hard to get enough breath to sing. Before and immediately afterwards I could sing perfectly! After about 2 years it started affecting my talking voice and last December i eventually decided to give the singing a break. Almost 10 months on and it's still the same! I read some posts on here about anxiety and I have similar symptoms to that! Constant nervous feel in my chest and feels like something stuck! I have spent over £3000 on hypnotherapy etc. And had a full physical all clear .

First, you need to see a specialist, not your primary care doc and a regular physical.  There are problems that affect the voice.  If it is anxiety, you would first be feeling nervous and anxious probably about singing.  It could be your posture, disc problems, all kinds of things that voice specialists would know about but your primary care doc wouldn't.
