Stress is a part of everyone's life. What you're describing is either an anxiety disorder or a deep insecurity, perhaps from something that happened to you. At any rate, I think it's time you see a psychologist for therapy to see if you can get to wherever it is you want to get in your life.
I'm on anxiety medication and that helps me, if you are able to talk to your doctor. If you're already on this, try things like keeping notes, paper or phone. I use an app called ColorNote, very very helpful to me. Color organization, calendar planning, locks for security. Just a good way to stay stress free.
