The CA-125 is not an accurate use for Endo however it can determine inflammatory diseases like, Endo, Adhesions, OVCA, Adenomyosis, inflammation, etc. Not one disease can pinpoint an elevated CA-125.
I had 3 done. Once during my period it was 37, once during ovulation it was 78 and after surgery it was 35. 35 and below is standard and with you having 45 its possible it could be anything but most likely Endo if you have those symptoms such as infertility. It is possible that endo is blocking your tubes and this surgery most likely will show that if you are having a hard time getting pregnant.
99.9% of cysts are benign so I don't want you to be worried. Multiple cysts on an ovary an indicated PCOS so not sure if they will be looking for that diagnoses as well. Were the cysts blood filled or regular? Blood filled cysts are called Endometriomas and if you have one it is an indicator of moderate to severe disease.
Before your surgery I would ask the following questions to your surgeon.
1) If Endo is found will it all be removed even if it is on the bowels and bladder and other places not just the tubes?
2) What is the surgeons experience with all stages of Endo?
3) Will he excise the endo (cut out) or laser which only gets the surfaces of the disease and causes more scar tissue.
4) will it be a basic laparscopic surgery or open surgery? If things get complicated will you continue or will another surgeon step in?
It is really important to know who is operating on you as I have met many women who have had multiple surgeries done because of lack of experience. I know one that had 22 surgeries by the time she was 26. that is insane. So Excision is the way to go and your first surgery is the most important.

There are only a handful of surgeons around the world that can properly remove the disease properly and I can give you the names to any that are around you.
I have many endosisters out there and the one thing that they say is they wish they had their surgery with someone with experience because they felt that it caused more problems with their fertility.
