multiple laps - where are incisions?

I had a lap when I was 16, where they went through my belly button and the two lower holes.  My last lap (15 years later), rather than go through my belly button, they made a one inch horizontal incision just below my belly button, plus the two lower tiny incisions.  My question is, can they not go through the belly button again once they went through there?  I'm trying to figure out why they didn't last time.
