2 months of treatment still having symptoms

Please keep in mind my boyfriend is Arabic does not speak English fluently and does not have any insurance.  I apologize that I do not know the names at this time of most of the prescriptions that have been prescribed. My boyfriend has never had unprotected sex with anyone but me and about 1 month after us having unprotected sex he started having a discharge mostly clear and a burning sensation when he urinated or ejaculated. Both of us were tested for sexually transmitted diseases and came back negative. But waiting for the test results of a STD my boyfriend was given a shot and a 1 doze pill assuming that he had a STD but when test results came back negative he was treated for Urethritis. (Which after reading info about Urethritis I do believe is considered an STD) After 1st treatment for Urethritis he still was experiencing symptoms and went to different doctor hoping that he had better insight and also my doctor that I trust.  This doctor also thought he had Urethritis and treated him with a different antibiotic - Doxycycline but again two weeks later no change in his symptoms he treated him with 2,000 mg of Flagyl  just in case he had Trichomonas and this was the culprit. Almost 2 months later he is still having these symptoms and appears to be getting worse and now his testicles are hurting in addition. Does anyone have any ideas on what this could be? I’m now worried about the amount of antibiotics he had consumed in the past two months. Could he have an antibiotic resistant that we do not know about since it is hard to communicate with him? Could he have Urethritis but needed to have a certain antibiotic for a longer amount of time then the 2 week dose or an Urethritis Perfusion treatment? Could he have Prostatitis? I feel so bad for my boyfriend and his language barrier is bad enough. I just want him healthy again. This has put a strain on our relationship. Also if I have given him a STD I need to be treated as well but my test results come back normal?

Thank you!!!!

Hello Dr. Kaul,

Thank you for responding to my post. I still have concerns for my now husband. He went to an Urologist and his dx is Prostatitis. It has been close to 6 weeks now and still having the same issues almost done with his Bactrim and Zyflamend. I asked the Urologist if Prostatitis was a STD his answer was no. But when I read info on line it appears to me that Prostatitis is indeed a STD. When we have intercourse using a condom he has less pain after then when we do not use one. If Prostatitis is sexually transmitted what else should I be checked for? And what is the super magic trick of getting rid of Prostatitis? I hope you can help him.

Thank you for your time!

Hi there!

Well, without a clinical evaluation it would be difficult to determine the cause of his symptoms. Urethritis per say is not an STD but refers to an inflammation or the urethra, which could be infectious (STD/ non-STD) in nature. With STD’s ruled out, possibilities that may need to be considered include infections not normally detectable on regular tests, inflammations, erosions, growths/ masses etc. I would suggest considering an evaluation by an internist or a urologist with possible reference to an infectious disease specialist for an accurate diagnosis and appropriate management.
Hope this is helpful.

Take care!
