Help with ultrasound results :) please!!

Ok i didnt really know where to post this so i hope this is OK!

:)

I'm a 19 yr old female and ive had on going problems with being tired all the time & headaches which cause me to feel a little nausia at times.
i sometimes have abdo pain and not sure why... long going problem as well.
- last period was a few weeks ago, i am on the pill so i normally like to choise to skip my periods of a month.
- recently had urine test showed blood - i couldnt see it when i did the sample.. they called it haematuria?
- recently have been going to the toliet alot. sometimes when i go only a little comes out and it hurts :( other times alot comes out. my bladder feels full alot.

2 days ago i had an ULTRASOUND of the URINARY TRACT the results say
No ureteric calculi were seen but these are not excluded on ultrasound.  There is a large amount of debris in the bladder as well as an echogenic rounded structure adherent to the wall of the bladder posteriorly measuring approximatly 2cm in diameter.

the conclusion of the ultrasound says,
bladder debris including a large adherent aggregate consistent with blood clot.  No ureteric calculus shown.  No other source of haematuria has been seen on this examination.


im very confused as to the result :(

Hi,

A UTI with common infective organisms would be detected on urinalysis, but infections with some sexually transmitted microorganisms, such as chlamydia and mycoplasma, are not detected on routine urinalysis or culture. These need specific antigen detection tests or culture media to identify their presence and sensitivity to antibiotics.

A blood test would identify whether you have any deficits in the hemoglobin or RBCs that could be causing a generalized weakness. Additionally, infection anywhere in the body would be picked up through an increase in the number of the WBCs.

Women who have been diagnosed with a UTI have been reported to nearly always contract another UTI within a year. Hence, it would be better to keep up your immunity levels with plenty of vitamin C, adequate fluid intake, and treatment of any anemia or other deficits.

Ive had a UTI previously in the past but none have shown up in any recent ones (recent being 2 weeks ago) so the dr sent me for a re test & a bunch of bloods to check for any deficiencys due to the tiredness (been on for years). tests previously have shown my kidney function is fine and the ultrasound also showed my kidneys were fine. a previous test mum mentioned and forgot to tell me about (i must of forgoten about it) said that one of my cell count was all mixed up and the dr wanted a re test?

  when i looked at my ultrasound scans the picture rather scared me due to the lump :(

i guess its just a wait to see what the results of this urine test and blood test shows.

Hi,

It appears that you have a urinary tract infection, specifically of the bladder. This is called cystitis and is consistent with the symptoms you have described and the ultrasound report.

Cystitis is caused by bacterial organisms that enter and colonize the bladder epithelium and can even spread to upper parts of the urinary tract, such as the ureters and the kidneys. If not treated early, it is likely that the damage to the epithelium would become so marked as to cause bleeding from areas in the bladder where the bacterial colonization has been more extensive.

Blood can be present in the urine in microscopic amounts and is detected on microscopic examination of the urine sample. The report mentions that there is considerable debris and this could be just due to the infection causing shedding of the bladder epithelial cells, WBC aggregation, small stones or crystals formed in the kidney, etc.

The adherent aggregate has bee supposed to be a blood clot that is still sticking to the bladder wall. Even though no stones were seen on the ultrasound, it does not rule out their presence and an Xray would be able to conclusively identify stones in the urinary tract, if present.

You will need to have a urinalysis and culture sensitivity done and go on a course of antibiotics, depending on which antibiotic the organism is sensitive to. Your GP should be able to prescribe the required antibiotics. Do start treatment as early as possible as long-standing infections can cause ascending infections that can induce complications resulting in impaired kidney function.

Ensure that fluid intake is over 2000 ml, avoid caffeine and alcohol, practice safe sex (condoms), pass urine after every sexual encounter, maintain hygiene, wipe front to back, and increase intake of citrus fruits and juices (lemon, orange, or cranberry) as these help to acidify the urine and eliminate the bacteria.
